Privacy Statement:
In consideration of my interest in employment with The University of Texas Health Science Center at Houston (UTHealth Houston), I authorize UTHealth Houston to make reasonable inquiries regarding any information I will provide in my application to UTHealth Houston (including, but not limited to criminal background check, verification of prior work history, verification of educational background, etc.) This consent also authorizes any agency or individual to provide the information requested by UTHealth Houston. This consent further authorizes UTHealth Houston to disclose the information collected to third parties as needed to comply with funding agency requirements and/or other contractual obligations.  I understand that if I am a male age 18-25, I must show either proof of registration with Selective Service or exemption from Selective Service at the time of hire. A reproduction of this request may be accepted as an original, and this acknowledgement and consent shall remain in effect until revoked by me. I understand that my application and accompanying attachments are the property of UTHealth Houston.
